Halo340B LLC (“Halo340B,” “we,” “us”) built this policy to be short and specific, because a policy nobody reads protects nobody.
What we collect
When you submit the assessment request form, we collect exactly the fields you fill in:
- First and last name
- Work email address
- Organization name
- Organization type
Our hosting and form-delivery providers also record ordinary technical information with each request, such as IP address, browser type, and the date and time. We do not use that information to build a profile of you.
What we don't collect
This site sets no cookies. It runs no analytics, no advertising pixels, no session recording, and no cross-site trackers. We do not buy contact data, and we do not attempt to identify visitors who don't contact us.
How we use what you send
We use your submission to review your 340B program, prepare your assessment, and follow up with you about it. We may keep a record of the exchange for ordinary business purposes. We do not sell, rent, or trade your information, and we do not use it for advertising.
Who else sees it
Service providers who make the site work: our form-delivery provider (Formspree), our website host, and our email provider. Each receives only what's necessary to perform that function. We may also disclose information where required by law, or in connection with a merger, acquisition, or sale of assets.
How long we keep it
We keep assessment requests for as long as needed to respond and to maintain ordinary business records, then delete them. You can ask us to delete yours sooner at any time.
Your choices
Email info@halo340b.com to access, correct, or delete the information you've given us, or to ask us to stop contacting you. We'll act on the request promptly. Depending on where you live, you may have additional rights under state privacy law; tell us what you're asking for and we'll honor it to the extent the law requires.
Security
We use reasonable administrative and technical safeguards to protect what you send us. No method of transmission or storage is perfectly secure, so please don't send patient information, protected health information, or other sensitive data through the website form. If you need to share something sensitive, contact us and we'll set up an appropriate channel.
Children
This site is intended for business use and is not directed to children. We do not knowingly collect information from anyone under 18.
